The Trinity Self-Directed Healthcare Plan offers a different approach to healthcare. Our plan offers a new way to manage costs for both employers and their employees.
Employees like the Self-Directed Plan because Trinity pays for eligible healthcare expenses first and the plan offers more control over where they receive care. The plan provides you with:
- Greater choice of physicians
- Direct access to specialists without a referral from a Primary Care Physician (PCP)
- A Health Reimbursement Account, funded by Trinity, which you can use to pay your covered expenses
- The ability to roll over any unused benefit dollars in your Health Reimbursement Account from year-to-year
- Eligible preventive care services paid at 100% by Trinity
- Healthcare coverage to pay for covered expenses after your Health Reimbursement Account is used up for the year and you've met your “Member Responsibility” amount
- Access to 24-hour Health Coaches, staffed by registered nurses and other healthcare professionals, to help you make the right healthcare decisions for you.
Dental Benefits:
Good oral hygiene is an important part of being healthy. Trinity offers employees dental coverage to (regular, full-time) employees and their eligible dependents. Coverage is generally based on services for preventive care, basic restorative services such as fillings, major restorative services such as bridges or crowns, and orthodontia, if covered. Most sites have one or two options available to select.
Vision Plan
The vision plan provides for an annual eye exam, prescription and frames. The plan is administered by Davis Vision.
Cancer (and Specified Diseases)
No one likes to think about having a serious health condition such as cancer. Programs like the Trinity Self-Directed Plan seek to encourage preventive care in order to improve your health, or to discover a problem early before it's serious. However, sometimes individuals are not able to prevent such diseases. Cancer and Other Specified Disease coverage can provide a source of funds in the event of an eligible, serious illness.
This is voluntary coverage offered at most locations. Employees pay the full cost of coverage. You may elect coverage for yourself or an eligible dependent.
Benefits may be paid for hospitalization-related benefits, extended care or hospice care, and other cancer related treatments. There are riders available that provide some benefits for preventive care.

Flexible Spending Account (FSA) for Healthcare expenses is available.
Coverage Effective Dates
Coverage eligibility commences on the 1st day of the month that immediately follows the employee's 31st day of employment.
Life and Disability Insurance
Accidental Death and Dismemberment (“AD&D”) Insurance
The Company provides $25,000 AD&D insurance for each eligible employee.
Short Term Disability/ Long Term Disability
Short Term/ Long Term Disability is a voluntary plan offered to weekly paid, non-union employees that provides income protection in the event of non-occupational injury or sickness.
Optional Employee Life/ AD&D and Dependent Life Insurance
Full-time/regular employees are eligible to purchase Optional Employee Life/Accidental Death and Dismemberment (AD&D) and Dependent Life insurance. Optional Employee Life/AD&D may be purchased in $5,000 increments-age and dollar restrictions apply.
Dependent life insurance also may be purchased. Dependent life insurance is offered in spouse/child units. You must elect employee life insurance to be eligible to purchase dependent life insurance. Your dependent life insurance may not exceed 50% of the amount elected for employee optional life.

Retirement Planning
401(k) Profit Sharing Plan
The 401(k) Profit Sharing Plan offers tax advantaged savings for retirement with two levels of benefits for employees.
Employees have the ability to save from one to fourteen percent of their pay beginning on the first of the month following a 60 waiting period. The Company may make a Profit Sharing Match on a portion of your eligible contributions after certain requirements are met.
Additionally, the 401(k) Profit Sharing Plan provides an enhanced benefit-an Annual Retirement Contribution (ARC)-that is funded 100% by Trinity. This Annual Retirement Contribution is automatic. You do not have to personally contribute to the 401(k) Profit Sharing Plan to receive the Annual Retirement Contribution. The amount of the contribution to your account is based on your years of service with the Company as of December 31 of each year.
Time Off Benefits for Drivers
Holidays
9 paid holidays per year. Eligible after 30 days of employment.
Vacation
Earned vacation time is determined by your length of continuous service as of your anniversary date.
- More than 1 year but less than 3 years = 40 hours
- 3 years but less than 10 years = 80 hours
- 10 years but less than 20 years = 120 hours
- 20 years or more = 160 hours
Funeral
Up to three days paid funeral leave for the death of an immediate family member. Coverage is effective on the date of hire.
Jury
Paid time off for the duration of jury duty. Coverage is effective on the date of hire.
Sick
Up to 10 weeks sick pay at half your normal hourly rate. Coverage is effective 6 months from date of hire.
